Functional MRI of human primary somatosensory and motor cortex during median nerve stimulation.

Abstract Objectives: Somatosensory evoked potential (SEP) studies suggested that some early cortical SEP components may be generated in the primary motor cortex (M1) rather than the primary somatosensory cortex (S1). Methods: We now used functional magnetic resonance imaging (fMRI) to study activation of S1 and M1 by electrical median nerve stimulation in healthy volunteers. Results: The hand areas of both S1 and M1 showed significant activation (correlation coefficients >0.45) in 7 of 9 subjects (activated volume S1>M1). Surgical therapy of recurrent vulvar cancer.

Objective The success of various surgical interventions in 201 cases with recurrent vulvar carcinoma was examined in the light of patients' pretreatment, surgical therapy, plastic reconstruction, and postoperative disease course. Study design A databank of standardized clinical data was analyzed using statistical procedures. Results Therapy was selected on an individual basis according to tumor status. Recurrence at a site distant from the primary tumor, particularly in the inguinal region, indicated a markedly unfavorable prognosis. Ulipristal acetate versus leuprolide acetate for uterine fibroids

A b s t r ac t Background The efficacy and side-effect profile of ulipristal acetate as compared with those of leuprolide acetate for the treatment of symptomatic uterine fibroids before surgery are unclear. Methods In this double-blind noninferiority trial, we randomly assigned 307 patients with symptomatic fibroids and excessive uterine bleeding to receive 3 months of daily therapy with oral ulipristal acetate (at a dose of either 5 mg or 10 mg) or once-monthly intramuscular injections of leuprolide acetate (at a dose of 3.75 mg). Ovarian sex cord tumor with annular tubules in a woman with premature ovarian failure.

Abstract Objective: To report a case of premature ovarian failure (POF) associated with an ovarian sex cord tumor with annular tubules. Design: Case report. Setting: Reproductive endocrinology unit in a tertiary academic center. Patient(s): A 20-year-old woman with POF. Intervention(s): Biopsy of the rudimentary ovary by laparoscopy. Main Outcome Measure(s): Protocol for POF investigation and histological study of the ovarian sample. Result(s): An ovarian sex cord tumor with annular tubules was detected in the rudimentary right ovary. Conclusion(s): The rare ovarian sex cord tumor with annular tubules, which may be hormonally active, was detected in a case of POF.
